Description
The position is responsible to assist the patient, their families and provider across all aspects of their health care continuum. The Care Services Coordinator is the liaison between the patient and medical provider and/or psychiatric consultant, with an optimum goal of meeting the patient's individualized needs, ultimately improving quality of life which will result in positive patient outcomes through communication, education and coordination. Is responsible for supporting and coordinating the mental and physical health care of with the patient's medical provider and, when appropriate, other mental health providersEssential Duties:
- Support the department with Care Services Coordination for patients under direction of medical providers and administrative functions under Department Management.
- Support the mental and physical health care of patients by closely coordinate care with the patient's medical provider and/or other mental health providers.
- Screen and assess patients for common mental health and substance abuse disorders using Mira Health Care Management software program.
- Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate ad directed by Medical providers.
- Facilitate patient engagement and follow-up care. Promotes timely access to all aspects of care.
- Promote adherence to care plans developed in coordination with the patient, Primary Care provider and family.
- Track and document in system, treatment response and monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.
- Provides excellent customer service.
- Implements care coordination and planning techniques.
- Participate in regularly scheduled consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient's medical provider. Consultations will focus on new patients and those who are not improving as expected under the current treatment plan. Case reviews may be conducted by telephone, video, or in person.
- Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant and who may need more intensive or more specialized mental health care.
- Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged.
- Assist patients in navigating through the healthcare continuum.
- Schedules appointments to ensure the patient is getting follow up with primary care provider and all specialists.
- Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).If patient requires referral to other services, will assure that authorization and referrals are completed.
- Coordinate appointments with other services/practices and advises patient accordingly.
- Obtain lab results, diagnostic studies for ancillary services; consult notes and other medical information in preparation for patient's upcoming appointment.
- Uses risk stratification tools to identify gaps in treatment
- Track patient follow up and clinical outcomes using a EHR and/or departmental documentation process. Document in-person and telephone encounters in the registry and use the system to identify and re-engage patients.. Document patient progress and treatment recommendations in EHR and other required systems so as to be shared with medical providers, psychiatric consultant, and other treating providers.
- Promote and/or provide patient education about common mental health and substance abuse disorders and the available treatment options as directed by the Primary care provider(s).
- Empower and educate patients to promote self-management and shared decision-making.
- Works collaboratively with providers and other care team members to ensure the delivery of quality of care to patients to help bring about best outcomes.
- Support medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.
- Communicates with a wide variety of people from a diverse socio-economic and ethnic background.
- Establishes and maintains effective working relationships with all team members.
- Engage with patients and caregivers to create an environment of collaborative care team.
- Follow up with patient as needed to ensure they understand their agreed upon goals; ie making all scheduled appointments, education classes
- Performs other duties as assigned.
Required Qualifications:
- Req High School or equivalent
- Req Bachelor's Degree In a related field
- Equivalent years of experience may substitute for Bachelor's degree.
- Req 1 year Experience in a related field in a healthcare setting
- Req Organization/time management skills.
- Req Excellent communication skills both oral and written.
- Req Must be comfortable with computers and medical terminology.
- Req Knowledge of HIPPA compliance and various health insurance types.
Preferred Qualifications:
- Pref Basic Life Support (BLS) Healthcare Provider from American Heart Association
Required Licenses/Certifications:
- Req Fire Life Safety Training (LA City) If no card upon hire, one must be obtained within 30 days of hire and maintained by renewal before expiration date. (Required within LA City only)
The hourly rate range for this position is $25.00 - $39.69.
